Receiving material from another institution
|
If you are interested in obtaining materials from institutions or commercial entities, please complete an Incoming
Questionnaire Form and submit it to the Office of Technology Management. This form provides essential information
about the provider, and the material that is being requested. The provider will usually send an agreement to be
signed by the investigator and an authorized representative. This agreement must be reviewed and approved by the
Office of Technology Management before any material can be received. In many instances these agreements place
certain contractual obligations on you and on Washington University. These terms may conflict with regulatory
guidelines, and the questionnaire serves to uncover these potential conflicts. We provide the questionnaires below
