If you are considering entering into any type of research collaboration with a company, contact us at the earliest possible opportunity so that we may provide you with the appropriate assistance. If your research includes the evaluation of pharmaceuticals or medical devices in human subjects, please contact the Center for Clinical Studies. Required Forms Depending on your school, please complete and submit the following forms to the Office of Technology Management before you enter into a research agreement of any kind. This is essential to protect intellectual property and to manage conflict of interests. Hilltop Campus and Medical School: Animal Welfare Assurance Form: A copy of this form must be on file with the OTM if the research project includes the use of animals. For more information, please contact The Animal Studies Committee at (314) 362-3229. Internal Review Board (IRB) Assurance Form: A copy of this form must be on file with the OTM if human subjects (including the use of human tissues) will be part of the research project. For further information regarding this form, please contact the IRB at (314) 362-3244. Medical School Only: Proposal Transmittal (PT) Form: Should be completed and signed by the Principal Investigator, then approved by the division, department and/or dean (see the approval section on the PT form). For further assistance in preparing the PT form, contact your department administrator. Budgets Your department administrator should communicate with the Grants and Contracts Office to ensure that the budget for the research project is entered on the Grants Budgeting system as soon as possible. A copy of the budget should also be on file with the OTM. Indirect Cost Waiver: Any research project budget that does not meet the required minimum indirect cost rate (50 percent Medical School or 53 percent Hill Top) must be approved by the Dean. A letter from the Dean approving the lower indirect cost rate must be on file with the OTM. It is recommended that an indirect cost waiver be obtained as far in advance as possible. Prior Approval/Project Activation Requests: Please forward the Prior Approval Request (Medical School) or Project Activation Request (Hill Top) to the Office of Technology Management for approval. Once approved, the Office of Technology Management will forward the request to Grants and Contracts or the Research Office as appropriate. That Office will establish a fund number for the project or extend the fund as requested. |
