It is particularly important that you identify collaborators who may have contributed to the invention; identify any proprietary materials that you may have used in the invention that were obtained from external sources; and list all forms of research funding support. When complete, sign the form and return the original to the Office of Technology Management. If you have explanatory information such as a pre-submission manuscript or a grant abstract, please submit that to assist us in thoroughly reviewing your invention disclosure.
